NOTICE: Special Form Certification for Model II and Model III SFCs were transferred to Southwest Research Institute on November 12, 2020 and December 9, 2020 respectively. Special Condition 4.b. of each certificate states that as of these dates, capsule components must be obtained from "either SwRI or an SwRI approved vendor." *None of these Special Form Containers can be purchased directly from OSRP. The cost per unit may vary depending on amount of items purchased from the manufacturer, cost/availability of raw materials, tax, shipping costs.

References, Information, and Limitations:

  • Any radioactive sources sealed within these Special Form Containers outside the presence of two or more fully trained OSRP personnel shall not be accepted into the OSRP recovery/disposal program and remain the responsibility of their owner.
  • Culpability for improper content loading, closure, use, or transport of the Special Form Containers remain the responsibility of the source owner or their broker.
  • All assemblers of Special Form Containers under certificates USA/0695/S and USA/0696/S must register their organization and QA program with the U.S. Dept. of Transportation as stated in Condition 5 of the special form certificate. For details and process, contact Mr. Michael Conroy, USDOT.
  • Assemblers are also required to obtain, maintain, and document function-specific training per 49 CFR 172.704.
