References, Information, and Limitations:
Any radioactive sources sealed within these Special Form Containers outside the presence of two or more fully trained OSRP personnel shall not be accepted into the OSRP recovery/disposal program and remain the responsibility of their owner.
Culpability for improper content loading, closure, use, or transport of the Special Form Containers remain the responsibility of the source owner or their broker.
All assemblers of Special Form Containers under certificates USA/0695/S and USA/0696/S must register their organization and QA program with the U.S. Dept. of Transportation as stated in Condition 5 of the special form certificate. For details and process, contact Mr. Michael Conroy, USDOT.
Assemblers are also required to obtain, maintain, and document function-specific training per 49 CFR 172.704.
